A 94.20% build also exists and many casinos use it; the info page inside the game shows which one you are on.
WHAT IT IS
Mental in short
Mental is Nolimit City's August 2021 asylum slot and the one most people mean when they say a Nolimit game is too much. Five reels in a 3-2-3-2-3 shape, 108 ways to start, a 66,666x cap, and a feature list that takes a page to explain. The published RTP on the default build is 96.08%, and a 94.20% build exists.
Canadians search for it because of the Mental Free Spins buy at 1,000x and the clips that come out of it, and because the art is grim enough that people want to see it once. It is a bonus-buy game first and a base game second. If you are not buying, the base game is a long wait with brief violence.
HOW IT PLAYS
Base game and symbols
Wins pay left to right on adjacent reels, any position, across the 108 starting ways. The grid shape means reels 2 and 4 show only two symbols until something opens the enhancer cells above and below them. Every base-game spin, between 1 and 13 positions get a Fire Frame. A Dead Patient symbol in a frame carries a multiplier from 5x up to 9,999x, and the frames are also the key: four frames open the enhancer cell on reel 2, six open the one on reel 4.
The enhancer cells hand out the three Nolimit mechanics. xWays turns a position into a stack of matching symbols and adds ways. xSplit wilds cut the symbols on their reel in two, doubling them. xNudge wilds nudge to fill the reel and carry a multiplier. The spider scatter on reel 2 or 4 can also transform into another symbol, which is how a near miss sometimes turns into a trigger.
Spins are quick and most of them do nothing beyond a frame or two. On C$0.40 a spin a C$30 deposit is 75 spins, which is not enough to expect a bonus. Most casino bonuses cap the stake at C$5 to C$7 a spin and exclude the buys, and some exclude the studio altogether; read the list.
Features
Fire Frames: 1 to 13 positions get a frame each base spin
Dead Patient multipliers from 5x to 9,999x
Enhancer cells on reels 2 and 4, opened by 4 and 6 Fire Frames, give xWays, xSplit or xNudge wilds
Mental Transform: a spider scatter on reel 2 or 4 turns into another symbol
Three rounds, picked by what lands. Three scorpion scatters give Autopsy Free Spins, 8 spins. Three scorpions plus a spider give Lobotomy, 9 spins. Three scorpions plus two spiders give Mental Free Spins, 10 spins, where the Dead Patient multipliers and the enhancer features all run at once and the 66,666x cap is reachable on paper.
The buys are 80x for Autopsy, 230x for Lobotomy and 1,000x for Mental, with a Lucky Draw at 232x that picks one for you. A dead round is 8 Autopsy spins with a frame or two and a multiplier that never meets a win, paying under 10x your stake. On a 1,000x buy, a Mental round that returns 200x counts as dead, and that is a normal outcome.
OUR TAKE
Should you play Mental?
Play it if you are buying bonuses with money you have already written off and you want the biggest, strangest Nolimit round there is. Skip it for a base-game session, for any bonus wagering, and if you want to understand what happened on a spin without replaying it. San Quentin xWays is the same studio with a more readable feature set, and Chaos Crew is the Hacksaw game for people who want the swings with a cleaner screen.

Only with money you are prepared to lose in one click. The 1,000x Mental buy at C$1 a spin is C$1,000, and the quoted RTP on the buys sits between 96.17% and 96.68%, which is not an edge. Most buys return far less than they cost; the average is held up by rare huge rounds.
02Can I play Mental with a casino bonus in Canada?
Often not. Many Canadian-facing casinos exclude Nolimit City games from bonus play, and all of them exclude the buys. If it is allowed, keep the stake under the usual C$5 to C$7 cap. It is one of the worst slots in any lobby for clearing wagering.
03What RTP does Mental run at?
96.08% on the default build, with a 94.20% build that many casinos use instead. The buys have their own figures, from 96.17% to 96.68%. Open the information page inside the game and check which base figure you are on before you deposit.
RTP, max win and release date checked against gambling.com, olbg.com in October 2026. Casinos can run a different RTP build; the game's own info page shows the one you are on.
